A brief description of the methodology used to track and calculate the number of recordable incidents of work-related injury or ill health :
An injured employee or student must report any accident where medical attention and/or first aid is required, or whenever any apparent bodily injury was received by the injured. All employees must complete a form CS-13 Employee Report of Accident or Injury, within 3 days after the date of the incident. Retain a copy for your files. Forward the original to your supervisor who will complete their portion. Supervisors are to forward the original to Human Resources, with a copy going to the Environmental Health and Safety Office (EH&S).

Forms SH-900 , SH-900.1. An incidence rate of injuries and illnesses may be computed from the following formula: (Number of injuries and illnesses X 200,000) / Employee hours worked = Incidence rate.

Incidence rate = 37 x 200,000/5270332.5 = 1.40

Binghamton University complies with federal, state and local safety and health standards, which includes OSHA, NYS Department of Labor and Department of Health, and Broome County Health Department standards. The Occupational Safety and Health Administration (OSHA) regulates workplaces of federal employees, as well as most others. The NYS Department of Labor regulates workplaces of New York State employees. There are standards for biological and radiological materials provided by federal and state agencies Hazardous materials regulations from the EPA and the Department of Environmental Conservation.
