A brief description of the methodology used to track and calculate the number of recordable incidents of work-related injury or ill health :
When an injury takes place that is work related, the Benefits Manager is notified. The appropriate steps are taken to make sure the injured employee is provided with the medical care needed. A work injury report is completed by the employee and/or supervisor that goes to HR. This information is passed to our Worker's Compensation Vendor and a claim is filed electronically. This data is also added to an excel spreadsheet as a log and provides the necessary information to help with completing the annual OSHA log that is due in January of each year of previous year results. Data recorded includes the following: name, department, title, date of injury, location of injury, injury type, injury description, race, hire date, dob, time reported to work, time of injury, days away from work, days on restricted duty, report injury report received, last day worked, whether treated in ER, etc. A file is also made with notes and forms related to the injury placed inside of the file and kept in HR.
