A brief description of the institution’s workplace health and safety initiatives, including how workers are engaged in monitoring and advising on health and safety programs:
A dedicated Risk Management & Safety Services Department has been established within the last year. The Campus Safety Committee has been re-instated. It is a communication venue for all employees to raise potential safety and health concerns at HSU. There are additional committees for health and safety at the department or unit level that report to the campus-wide safety committee.

The performance and baseline year data inserted is directly from Humboldt State University's OSHA 300A forms from 2010-2015. The information is collected within the Workers' Compensation department, which is housed in the Human Resources department. The Total Employees or FTE information is calculated by our Payroll department.
