A brief description of the methodology used to track and calculate the number of recordable incidents of work-related injury or ill health :
The Risk Management team collects and investigates claims for personal injury and property, negotiates settlement of theft and property damage, coordinates loss prevention and risk reduction actions, works closely with each Member’s Risk Management Contact, develops proposed written policy and procedures, manages the central data system, and initiates third party recovery and subrogation of property damage claims. Origami is the data portal used for reporting, recording, and evaluating injuries and safety incidents. Annual number of recordable incidents per 100 FTE employees is calculated using the OSHA formula for Recordable Incident Rate.

The FTE number provided in the credit differs from PRE-5 because injuries are tracked at the system level, and they generate their numbers with a different methodology than we do internally on campus through Human Resources at the campus. We also use a different methodology for FTE calculations as provided by System HR. The formula we use is the OSHA Recordable Injury Rate: # incidents * 200,000 / total number of hours worked by employees in one year. For us, that calculation was 159 * 200,000 / (2032*11,872) = 1.318 because there were 2,032 working hours in FY 23.
