A brief description of the methodology used to track and calculate the number of recordable incidents of work-related injury or ill health :
An injured employee is required to report the injury to management as soon as it occurs. The injured employee completes four Workers Compensation forms. The supervisor completes a “Supervisor First Report of Injury” and if any witnesses, they will complete a “Witness form”.
The supervisor has 24 hours to complete the form from when he/she is first notified and the injured employee has 48 hours to complete the forms.
All W.Comp forms come to me and I file the incident electronically to State Office of Risk Management. SORM generates a claims number and assigns an adjuster.
